Globus pharyngeus, also called globus sensation or globus, is the feeling of having something stuck in the throat. It is an overwhelming feeling of a lump or foreign object being lodged in a person’s throat.

It is common, with 22–45% of people experiencing it at least once in their lifetime.

Globus pharyngeus causes

Globus pharyngeus is often caused by inflammation of the throat. Exact cause behind globus pharyngeus is unknown. Factors which are connected includes:-
Post nasal drip.
Gastro-esophageal reflux.
Lingual tonsils.
Cricopharyngeal spasm.
Enlarged thyroid.
Hiatus hernia.
Chronic sinusitis.
Cervical osteophytes.
Foreign body such as fish/chicken bone.
Laryngo-pharyngeal cancer.
Anxiety

Globus pharyngeus diagnosis

Diagnosis of globus pharyngeus is typically a diagnosis of exclusion.

A detailed medical and personal history is taken which includes dietary habits, sleep schedule, history of smoking and alcohol intake and any recent stressors in life. Next step is a physical examination which includes a general physical examination, detailed evaluation of ear, nose and throat. Once clinical examination is over, few differential diagnoses are shortlisted and tests advised based on these. These may include blood tests, endoscopy, X-rays and ultrasound of the neck initially and CT or MRI scan eventually.

Reassurance of the patient is recommendable when no cause can be found.

What is the meaning of Ct value in the COVID test?

If a cause is identified, treat the cause symptomatically or, if possible, systemically.

Globus pharyngeus Differential diagnosis

Differential diagnosis must be made from Eagle syndrome which uses the patient’s description of “something caught in my throat” as a diagnostic tool.

Trigeminal neuralgia is a rare nerve disorder that affects the face. It causes sudden and severe pain in the face, which people describe as a shooting, electric shock-like pain in the teeth, jaw, gums, and other areas of the face.

SYNONYM

It is also known as tic douloureux. The pain is typically focused on the lower face and jaw, although it sometimes affects other areas of the face, such as around the nose and above the eyes.

Trigeminal neuralgia cause

Trigeminal neuralgia is caused by irritation of the trigeminal nerve, which starts at the ear and branches out to the forehead, cheeks, and lower jaw. It usually limited to one side of the face and can be triggered by actions such as eating or brushing.
It is a type of neuropathic pain, which stimulates the fifth cranial nerve.

Trigeminal neuralgia Treatment

Antidepressant medicine such as amitriptyline or nortriptyline, are effective in treating nerve pain.
Antiseizure medications such as carbamazepine is effective for trigeminal neuralgia.
Short-term narcotic pain medications, such as codeine can be used.
Microvascular decompression (MVD) surgery is regarded as the most long-lasting treatment for trigeminal neuralgia caused by blood vessel compression and it helps about 80% of people with this diagnosis.
Stereotactic radiosurgery is a fairly new treatment that uses a concentrated beam of radiation to deliberately damage the trigeminal nerve where it enters the brainstem. Stereotactic radiosurgery does not require a general anesthetic and no cuts (incisions) are made in your cheek.

Ct value stands for cycle threshold. During the RT-PCR test, the Ct value determines the detectable level of the virus in a given sample based on which it is decided if the person is COVID positive or negative.

In RT PCR test, the RNA is extracted from the sample and is converted to DNA, which is then amplified or multiplied. This multiplication happens over a number of cycles in order to find out the viral load.

If the virus is found within fewer cycles, it mean that the body has a high viral load.

If the virus is found after more cycles, this means the body has lesser viruses which could only be detected after the sample was passed through many cycles.

In this way, the Ct value is inversely proportional to the amount of viral load in the human body.

Higher the Ct value lesser the viral load and lower the Ct value, higher the viral load.

A higher viral load is associated with being more infectious.

Interpretation of Ct value in COVID test A patient is considered Covid-positive if the Ct value is below 35.

In other words, if the virus is detectable after 35 cycles or earlier, then the patient is considered positive.

Any correlation between a Ct value and severity of COVID?

Although Ct value is inversely correlated with viral load, It does not have any bearing on the severity of the disease.

A patient can have a low Ct value, which means her viral load is high enough to be detected rapidly, but she may still be asymptomatic.

The Ct value tells us about the viral load in the throat and not in the lungs.

Does a high Ct value always mean a low viral load?

Factors that may vary the results of Ct value includes:-

A poorly collected sample may reflect inappropriate Ct values.
Technical competence of the person performing the test.
Calibration of the equipment.
Analytical skills of the interpreters.
Ct values may differ between nasal and oropharyngeal specimens collected from the same individual.
The temperature of transportation of the sample.
The time taken from collection of the sample to receipt in the lab, can also adversely impact Ct values.

A sore throat is pain, scratchiness or irritation of the throat that often worsens when you swallow.

  • The most common cause of a sore throat (pharyngitis) is a viral infection, such as a cold or the flu. Interestingly, a sore throat caused by a virus resolves on its own.
